Prior to surgery, a Coronavirus/COVID-19 Screening questionnaire will be completed on your preoperative phone interview and upon entering the building on the date of surgery.

Should you answer yes to any of the following, your procedure will be postponed.

1. Have you and/or anyone accompanying you today traveled outside the U.S.A within the past two weeks?    

2. Have you and/or anyone accompanying you today traveled outside PA to another state within the past two weeks? 

3. Have you and/or anyone accompanying you today been in close contact with anyone who has traveled to these areas (Outside the USA or PA) in the past two weeks?  

4. Have you and/or anyone accompanying you today been in close contact with a person confirmed to have COVID-19

5. Do you and/or anyone accompanying you today currently have a fever or respiratory symptoms such as a cough or shortness of breath, fever of 100.4, rash, headache , muscle aches, loss of smell or taste, chills , repeated shaking with chills, sore throat.

Integrated Surgical Institute has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

  1. Social Distancing practices of 6ft separation.

  2. Temperatures will be taken upon arrival to the building. If you have a Temp. over 100.4 you will be not be admitted to the facility and referred to your physician.

  3. A face mask will be required while in the building, except when medical procedures require a mask be removed.

  4. Hand Sanitizer stations will be available for use in each area of the facility for frequent hand sanitizing.

  5. All patients will be asked COVID 19 screening questions

  6. Only patients will be admitted to the facility. If you are a minor, an adult will be admitted to the facility to accompany that patient. The accompanying adult will also be required to be scanned for temp and complete a COVID 19 screening questionnaire.

  7. All accompanying adults will be asked to wait in their car or be available to pick the patient up upon discharge via a text message or phone call and to stay within 5-10 minutes of the facility location.
